About us

Two specialist nurses helping people plan ahead for their future care, with the time, knowledge and independence to get it right.

Who we are

We are an independent, nurse-led advance care planning navigation service. We help individuals, families and healthcare professionals understand the choices available for future care and treatment. We then work with them to make sure those choices are clearly recorded and understood.

Between us, we bring decades of experience in palliative, oncology and haematology nursing, and in leading a national nurse-led end-of-life helpline. We combine that clinical knowledge with practical, personalised support.

Our work goes beyond filling in forms. We help people explore what matters to them, think through how their decisions might apply in real clinical situations, and express their wishes in a way that will be clear and meaningful to those caring for them when difficult decisions arise.

    Co-founder · Registered nurse

I’m a registered nurse with nearly 20 years’ experience in healthcare, beginning in oncology, haematology and end-of-life care at Guy’s and St Thomas’. I then moved into specialist helpline nursing at Prostate Cancer UK and Compassion in Dying, where I spent almost a decade and went on to lead the nurse-led helpline as Deputy Director. Along the way I supported people directly, led specialist services and delivered education and training to healthcare professionals through conferences, webinars and teaching.

Much of my career has involved helping people understand their rights and choices, explore their options and navigate difficult decisions about treatment and care. I’ve seen first-hand how confusing and overwhelming the healthcare system can be, particularly when people’s wishes aren’t understood or they don’t feel involved in decisions.

Working outside the clinical setting gave me a different perspective. With health and care colleagues under extreme pressure, there is often little time for these conversations. I saw how valuable it is to have someone take the time to listen, explain complex clinical and legal information in plain language, and help people feel more confident speaking with healthcare professionals.

I co-founded the Advanced Care Planning Navigation Service because I believe in informed choice. Everyone deserves clear, balanced, clinically informed support to make decisions about their care, and to have their wishes understood and respected.

Co-founder · Registered nurse and Independent Prescriber

With over 25 years of nursing experience, I have built my career around supporting individuals and families through complex healthcare decisions, with specialist expertise in palliative care, advance care planning and clinical decision-making.

My professional background spans frontline clinical practice, senior specialist nursing and clinical leadership. As an Independent Prescriber, with additional advanced clinical training, I bring extensive clinical knowledge, a strong understanding of healthcare systems and considerable experience in supporting people to navigate difficult and often sensitive decisions.

Throughout my career, I have seen first-hand how challenging it can be to understand healthcare options, access appropriate support and ensure individual wishes are recognised and respected. I am passionate about bridging that gap, providing clear, evidence-based information and personalised guidance that enables people to make informed decisions with confidence.

I believe that everyone deserves to feel informed, listened to and empowered when making decisions about their health and future care.

My approach combines clinical expertise, professionalism and genuine compassion, with a commitment to ensuring that every conversation is guided by what matters most to the individual.

Our mission

Our mission is to empower individuals, families and healthcare professionals with the knowledge, guidance and confidence to navigate complex healthcare decisions and plan effectively for future care.

Through independent, specialist nurse-led expertise, we aim to bridge the gap between clinical knowledge, healthcare systems and meaningful, person-centred decision-making.

We are committed to providing clear, evidence-based information, professional education and personalised support, enabling individuals to make informed choices while equipping healthcare professionals with the knowledge and confidence to facilitate sensitive and complex conversations surrounding advance care planning.

At the heart of our service is a commitment to improving understanding, strengthening confidence and ensuring that advance care planning decisions are informed, considered and guided by what matters most to the individual.

Our vision

Our vision is a future where everyone, regardless of their background, circumstances or socioeconomic status, has equitable access to the knowledge, guidance and support needed to make informed decisions about their health and future care.

We are committed to addressing health inequalities, with a particular focus on communities experiencing socioeconomic disadvantage and those who face additional barriers to accessing healthcare information, support and services.

Through collaboration with individuals, families, healthcare professionals and community organisations, we aspire to improve access to specialist knowledge, strengthen understanding and promote meaningful involvement in healthcare decision-making.

We recognise that achieving equity requires more than simply making services available. It means understanding the diverse needs of communities, recognising the barriers people experience and developing accessible, inclusive and responsive approaches to support.

Our ambition is to contribute to a more equitable and person-centred healthcare system, where every individual has the opportunity to understand their options, express their wishes and make informed decisions that reflect their values, preferences and priorities.
